Forms

Home School Agreement
Each year every parent and child will receive a copy of the Home School Agreement which they need to read, sign and return to school. The agreement explains what we as a school will do for each pupil and also outlines what we expect from each child and parent. The form is circulated in the first half term of the year or in cases of mid-year admissions, the form will be issued shortly after they begin. A blank copy of the form can be viewed below. Leave of absence 
Holidays should be taken outside the school term. Children can miss important aspects of work if they are taken out of school.
In conjunction with amendments to the government's policy on granting leave of absence, from September 2013 Headteachers will not be authorising school holidays during term time.

There may be occasions where parents/carers feel that their circumstances are exceptional, being of unique and significant emotional, educational or spiritual value to the child which outweighs the loss of teaching time, or there are other difficult circumstance within the family. Each application will be considered individually and may require a meeting with the Headteacher.
Leave of Absence Application Form

If you wish to apply for "Leave of Absence", please return the form to school at least four weeks before you are due to take your absence in order for your request to be considered.

​Medication 
If your child has been prescribed medication by their doctor, we are able to administer this with prior consent from the parent.
​Please complete the Administration of Prescribed Medication form and return it to school with your child's medication.
